Sharing What Works: Proceedings from the MSHO Annual Meeting
The NCCC has published proceedings from the Minnesota Senior Health Options (MSHO) 2001 Annual Educational Forum. The MSHO demonstration facilitates the integration of primary, acute, and long-term care services for people aged 65 and older who are dually eligible for Medicare and Medicaid. The forum is one of the resources provided by the NCCC as the demonstration's Technical and Educational Assistance Program (TEAP) provider. Some of the topics presented at this forum include: national and local perspectives on care coordination, high-tech and low-tech solutions at home, improving transitions between settings, and disability prevention in the senior population.
