Partnership for Solutions Makes Case for Chronic Illness Care Reform
The Partnership for Solutions, led by Johns Hopkins and The Robert Wood Johnson Foundation continues its efforts to
improve the care and quality of life for people with chronic health conditions. One of the Partnership's major activities
is to communicate research findings to policymakers, advocates, health professionals, and business leaders. Two new fact
sheets provide a compilation of the most salient data available on the need for chronic illness care reform. Multiple
